What do I put if I don't have a company name on your mechanics lien template?

I was simply hired by the tenant as a subcontractor, not associated with any company at all. What do I put on the template since it states it will show up on the paper?

Most mechanics lien claimants will be a construction company, a supplier, an equipment company, etc. But, individuals can also file mechanics liens if they've provided lienable work in their own capacity.
