I would think LevelSet could generate a template for you in that regard, though I have not verified that, but I do want you to be aware that if you are a second-tier contractor (or "sub-subcontractor) you will need to send your notice by Certified Mail to the prime contractor within ninety (90) days after the date on which your company last provided labor or materials for which the claim is made. I recommend CC'ing the subcontractor that you contracted directly with and the surety in question as well.
