What type of lien would I file if I wanted to loan my elderly grandmother money for care giver expenses against the sale of her home when she passes?

You could use a note tied to a deed of trust which would be recorded on the property. The note will reflect the money being lent to your grandmother. And the deed of trust is a document that is recorded against the property which secures the note obligation.
