What information do we need to provide in our Lien Waiver for our worker?

We are finishing a job as a subcontractor on a project and are finishing it up within the next week. We would like our worker to sign a Lien Waiver on the project. What form should we fill out in order to avoid any issues once we make final payment to our employee. Thanks, Dom

Lien waivers in Illinois are mostly unregulated. There is no explicit statutory language or specific form requirements that must be met in order to be valid. However, a typical lien waiver should include a few things. For one, it should identify all the relevant project information such as; the lien claimant's information, your information, the project location, and the owner's information. Along with that, the lien waiver should identify the amount being paid and a statement declaring that lien rights are being waived in exchange for final payment.
