What are my lien rights with respect to a change order that my client refuses to approve? Do I have to bill them for the disputed amount? Can then lien amount only be for the contracted amount or can I include the change order as well?

It really depends on your individual situation, but generally, it makes sense to bill for the disputed change order. The billing for the disputed change order becomes part of your "contract amount" and your lien should generally include the amount of the change order.
