Generally, it's not a good idea to submit documentation that isn't accurate. And, signing a document you know to be false is never a good idea. Instead, it's typically a better move to either edit the document so that it's accurate or to find some other solution. To clear things up about what's expected and what documentation should be submitted, it'd be a good idea to discuss the matter with all the relevant parties involved.
