how many valid lien forms are there in Louisiana

good morning, we have a subcontractor for a project in Bossier city, LA, that says he has recorded a lien on the property, it is not on the same form as the Louisiana Statement of Claim and Privilege Form on your website, are there other acceptable forms, the copy they sent us has the Clerk of court name, a number, a recording date and says LIEN, then has some wording in the body of the form, i could e-mail a copy if it would help, thank you, Tom

The actual language on a mechanics lien claim can vary in Louisiana, as long as all of the statutorily required information is provided. The specific requirements can be found under LA. Rev. Stat. §9:4822(H)

A statement of a claim or privilege:
(1) Shall be in writing.
(2) Shall be signed by the person asserting the same or his representative.
(3) Shall contain a reasonable identification of the immovable with respect to which the work was performed or movables or services were supplied or rendered.
(4) Shall set forth the amount and nature of the obligation giving rise to the claim or privilege and reasonably itemize the elements comprising it, including the person for whom or to whom the contract was performed, materials supplied, or services rendered...; &
(5) Shall identify the owner who is liable for the claim under R. S. 9:4806(B)...

Thus, if all of this information is provided (along with any other parish-specific recording requirements) the claim will be valid and accepted for filing. The clerk of court name, recording date, etc., is added to the form by the recorder upon filing of the claim to provide all of the relevant recording information.
