Unfortunately, I am not sure of exactly what you are trying to ask but I will still try to answer.

No, there is not a form or anything else you need to put a subcontractor on notice that you are hiring an additional subcontractor. You are the GC. You hire who you need to complete the job.

HOWEVER, if you are going to reduce the amount of pay the first subcontractor receives because the second subcontractor is finishing or doing part of their work, you will need to square that up with the first subcontractor or you may find yourself on the wrong side of a breach of contract lawsuit.
