Calif Preliminary 20-Day Notice Question

I am in the process of filling out a 20-day preliminary notice and was wondering if I am able to include a separate sheet for the area where I describe the labor, services, and equipment/materials? The 3 lines on the form are not enough room to include all the information I need to include.

The description of the scope of work is not intended to be extensive. There is nothing wrong with providing an attachment, but if you can describe the scope generally, that is usually considered acceptable. I suggest you contact an attorney familiar with mechanic's liens to assist you.
