6. Provide the first and last dates that work on the project was performed. These terms are also known as the first and last dates of
performance.
7. Next, you’ll need to include the property’s legal description. You can find this information by requesting it from the Suffolk County Clerk’s office.
8. Lastly, get your lien notarized, witnessed, and signed.

After the lien is recorded, you have 30 days to serve the property owner with a copy.
