Include a statement for the labor and materials furnished for the construction project.
Include the lien amount based on the value of labor and materials you furnished for the project.
Provide the first and last dates of performance of work or furnishing of materials for the project.
Include the property’s legal description.
In the state of New York, lien claims must be witnessed and notorized.

After that’s done, you’re required to serve the property owner with a copy of the affadavit within 30 days of filing. Otherwise, the lien is rendered invalid.

What are the margin & page size requirements to file a lien in Nassau County?
