I need a form which allows me to notice a lien notice with 10 day notice and need that form and actual lien to be filed

To download a Colorado Notice of Intent to Lien click here. Keep in mind that this must be sent at least 10 days before filing a mechanics lien. Sending this notice will not extend the deadline to file a mechanics lien, so the earlier the better.
In addition to that, the Colorado Notice of Intent to Lien requires that a completed Lien Statement be sent with the notice as well. Those forms can be found at these two links:
