The Intent to Lien form Format is WRONG

I went to use the Intent to Lien Form and the format is not correct

In Colorado, while a Notice of Intent to Lien is a required document, the Colorado Revised Statutes doesn't actually set out a certain form or format for a Notice of Intent to Lien. So, while a format may seem unfamiliar, the format looking different from another Notice of Intent to Lien document found elsewhere might look a little different - and both may be absolutely valid. Now, there is certain information that must be included on the form - and if some of that information seems to be incorrect or missing, then that would certainly need to be addressed.
