Statutory Lien Waiver Forms

I read that 12 states need to use the specific lien waiver form including the states where we do business. We use a software that generates lien waivers, and while it doesn't generate the form, it is worded exactly the same for each respective state. Is this OK to use, or do we need to use the form?

Well your giving up your lien rights for receiving payment from the client, so as long as the form clearly expresses that then I don't see what the problem is. I've never heard of a contractor being disciplined for using their own form. You can always contact the CSLB directly and ask them. Either way should probably have a Construction Law attorney review your form.
