Is there a different lein form for private party judgments vs construction (business) judgments

I need to file a lein against a party that owes me for work and materials supplied for the job that was never completed. It was filed in MN and he subsequently sold his property here and moved to AZ (his primary residence) I have an authenticated judgment and I have to file a foreign judgment to move this MN judgment to CO. where he has part time residence. Once I register this lein with the clerk of courts in CO how is he notified of this lein?
