I often hire fabricators to build fixtures off-site and then bring them on-site for final installation, outside the scope of a General Contractor. Because I have hired them directly and have receipt of payment, do I need to use a lien waiver?

It's a good idea to collect mechanics lien waivers from anyone who's provided labor or material to the project. But, as you mention above, if you've got proof the fabricator has been fully paid (like a copy of a check or a bank statement) - then obtaining a lien waiver might not be necessary. After all, once they've been paid, they won't have lien rights.
Still - for peace of mind, it's typically a good idea to collect waivers anyway. Plus, if there is a dispute later on over the price of services provided, etc., then having a final lien waiver on hand would be extremely valuable.
