How to strike a lien for failure to serve

A fraudulent mechanics lien was placed on my property, and I was never notified. I do not know what form(s) to fill out to have the lien stricken as per Pennsylvania Statutes Title 49 P.S. Mechanics' Liens § 1502. Filing and notice of the filing of a claim. I have tried to file online several times, but it keeps getting rejected by the prothonotary, for having the incorrect form.

Generally, under § 1505 of Pennsylvania's mechanics lien statute, challenging a mechanics lien for failure of service must be done by preliminary objection. While I've got little experience in that department, here's a downloadable guide to filing preliminary objections in Pennsylvania, which should be useful. Additionally, or alternatively, it might be helpful to consult a local attorney for information on how, exactly, to file a preliminary objection to an improperly filed mechanics lien claim.
