How to present costs for unforeseen conditions on progress Pay Apps with set contract amounts. Can CO be for time only.

I am reviewing Best practice for tracking expenses related to unforeseen conditions, including Change Orders, schedule, and representation of costs on progress Payment Applications. Especially from Architect/QA standpoint.
