how to fill out form correctly - name of claimant

We are a supplier only - product sold direct to subcontractor. There has now been a disagreement between the sub and general contractor -we want to get paid directly - when filling out a Conditional Lien Waiver form - would we be the "Claimant"

Generally, on lien waivers, the "claimant" will be the party who is submitting the lien waiver, thus waiving their right to make a claim. However, I'm not able to provide legal advice or to instruct you how to fill out any particular form.
