Prime contractor has two subs listed on their lien release

Is this for a Conditional/Unconditional Waiver & Release form or an actual Release of Lien form? I’ve never seen a GC do this before. If it’s for a Release of Lien (where the lien has already been filed), it should include the name of the lien claimant. If there are 2 liens, there should be 2 Release of Lien forms. If it’s for a Conditional/Unconditional Waiver & Release form, there really should a separate form for each sub as well. The law doesn’t say anything about using one form for 2 subs.
