About Michigan Bond Claim Form

This is a Michigan Bond Claim form for use when a subcontractor or supplier is unpaid on a public project in Michigan and would like to file a claim against the project’s payment bond.

A Michigan Bond Claim form must be sent by anyone who didn’t contract with the general contractor, no later than 90 days from the last date of furnishing labor and/or materials to the project. The claim should be sent by certified mail to the prime contractor and the public entity who commissioned the work.

Pro tip: Those who did not contract with the prime contractor must deliver preliminary notice within 30 days of first furnishing labor and/or materials on the project to retain their right to file a bond claim.
