If that is the case then you're in luck!  The answer is simple because Arizona is one of 12 states that regulate the lien waiver forms. This means that Arizona lien waivers must always follow the same format and contain the same information (as required by law).  So, it doesn't matter if you get the form template from the other party, or you provide it yourself, it will be the same format/form.
