If you were looking for some other waiver form or template - I'm afraid I'm not sure that's something Levelset will have available. There are a number of different waiver of liability forms available elsewhere online - but, it'd likely be wise to narrow down your search a bit to identify the specific type of liability you're looking to have waived.
