is there a specific lien form for architects and design professionals

If you're referring to the form for a mechanics lien (which may be filed by an architect and other design professionals): the California mechanics lien statute doesn't specify a particular form that must be used, and the requirements for what must appear on a lien claim are the same for an architect and design professionals as they would be for any other claimant. You can find all of those requirements at § 8416 of the California Civil Code. If you're referring to a California design professional's lien, there's specific information that must be included in a design professional's lien, pursuant to § 8304 of the California Civil Code. Such a lien must include "(1) The name of the design professional. (2) The amount of the claim. (3) The current owner of record of the site. (4) A legal description of the site. (5) Identification of the building permit or other governmental approval for the work of improvement."
