Does back of Pre-Lien require a signature?

ArizonaPreliminary Notice

Our owner for a project in AZ said she received our pre-lien notice but asked if our CEO needed to sign the back or if it is just for her records? I'm not familiar with this but is there a signature point on the back of this notice? Does she need to do anything with it? Thank you

That is an open question. If the preliminary 20-day notice identifies who is sending it, probably not. The statutory form has a place where a signature is supposed to go, but there is nothing in the statute that says a signature is required, and Arizona law says that a signature does not necessarily require that someone put pen to paper. To be on the safe side, it is better to have a signature so that this does not become a potential issue down the road.
