Does a Contractors License Holder have to have a Business name?

I was a RME for a Corp which closed and the license was canceled. I now want to reinstate my license without testing. Do I need to have a company or can the license simply be in my name? Also do I need to recertify my experience again? I understand I don't have to retest, since I'm in the 5 year window and to use for 13A-1A, but not sure about the questions I've asked.

If you were already a qualifier you probably do not have to take the test again, so long as the license upon which you were the RME has not yet become void. A license becomes void after 5 years of not paying dues. Go to CSLB.CA.GOV to apply.

Thanks Brion. I do not need to retest and there is a form for me to use. My real question is the form asks the name of the business. I just want the license in my name since I'm not sure how much I'll use it. If I start getting some work I can always get a business license. The second part is do I need to recertify my experience since I already qualified previously.
